As seen in the screenshot, the from fields should be editable in their position.

The note dialog, is for stiky-notes annotations (not form fields)

When I'm touching form field there is no keybord for editing value. Combobox fields are also unresponsive.

We have tested also on 5.1, no problem.
It seems like the plugin is not updated correctly, try to recreate the cordova project
